Payment of benefits
All benefits are payable to the Member or nominated beneficiary (where the Member is the deceased) in accordance with the group scheme Policy.
The benefit payable is selected at scheme level and the Employer must pay all contributions for the cost of this benefit.
A Family Funeral Benefit Nomination form must be completed by all members at commencement of the policy and must be retained by the Employer.

Eligibility
Membership is compulsory for all Members who satisfy the eligibility conditions.
The maximum entry age for spouses is 65 years.
Children will be covered up to age 22, unless they are studying full-time at a recognised school or tertiary institution, then cover will continue until age 26.
Cover will be provided for a maximum of the 4 youngest children

Exclusions
Exclusions applicable to the Family Funeral Benefit are:
Active participation in war, riot and civil commotion, terrorism,
all claims related to atomic, biological and chemical warfare or terrorism,
the commission of a crime, suicide, attempted suicide or self-inflicted injury and deliberate exposure to exceptional danger will not be covered during the first year of membership from the date of receipt of the first month's premium.

Notification period
At the death of a Member, Spouse or child, the Employer must notify Global Alliance Life within 1 month from the date of death.

Submission of claim documents
The Employer should submit all required claim documents to Global Alliance Life within 1 month from date of death.
Claim documents to be submitted

Main Member:
Completed, signed and stamped claim form,
Originally certified copy of deceased's identity document or, passport,
Originally certified copy of the death certificate,
Copy of premium schedule,
Proof of membership,
Beneficiaries banking details – if Global Alliance are not paying the Employer,
If cause of death is unnatural – a copy of the police report is required.

Spouses Benefit:
Completed, signed and stamped claim form,
Originally certified copy of deceased's identity document,
Certified copy of the death certificate,
Certified copy of the main member's identity document,
Certified copy of the marriage certificate. If not available: a copy of the “lobola” letter is required,
Certified copy of the Nomination of beneficiary form,
Copy of premium schedule,
Beneficiaries banking details – if Global Alliance Life are not paying the Employer,
If cause of death is unnatural – a copy of the police report is required.

Children's Benefit:
Completed, signed and stamped claim form,
Certified copy of deceased's identity document or birth certificate,
If no birth certificate is available – a copy of the clinic card is required,
Certified copy of the death certificate,
Certified copy of the main member's identity document,
Certified copy of the Nomination of beneficiary form,
Copy of premium schedule,
Beneficiaries banking details if we are not paying the Employer,
If cause of death is unnatural – a copy of the police report is required,
If the child is over the age of 18 – we require a certified copy of a letter from the school confirming the child's registration and current grade that the child was attending.
