Permanent Disability Benefits
Outstanding amount of loan
A Permanent Disability benefit is payable at the first occurrence or manifestation of a medically diagnosed impairment or condition which totally and permanently prevents a Member, from following his own occupation and any other occupation. Such impairment or condition must have endured totally and continuously for a period of at least 6 months from the date of injury or the onset or manifestation of the illness or disease.
Total and Permanent Disablement shall be deemed to exist if the Member suffers the loss of use of any combination of two of his hands or feet.

Total Temporary Disability Benefit
Monthly loan payments
The benefit is payable if the Member is totally and temporarily prevented, from following his own occupation and a similar or other occupation for which he is suited having regard to his ability, training, education and experience from earning his normal income. Global Alliance Life shall pay, after a waiting period of 30 days, the Total Temporary Disability Benefit, or a portion thereof if the Member suffers only a partial loss of income, until the recovery of the Member.

Payment of benefits
Benefits are payable to the Creditor / Loan Institution.

Eligibility
The maximum age for a Member to join the group scheme is 65.
A group scheme Member is allowed to enjoy Group Life Benefits up to a maximum age of 70.

Exclusions
Exclusions applicable to all the benefits are:
Active involvement in any war, riot or civil commotion related incident, terrorism or atomic, biological or chemical warfare or terrorism. No Group Life Benefit claim will be paid if death arises directly or indirectly or is traceable to suicide within the first 24 months of membership of the Scheme. No Critical Condition Benefit, Total
Temporary Disability benefit or Permanent Disability benefit claim will be paid if the claim is directly or indirectly caused by or attributable to:
the Member's commission of a crime; or attempted suicide or any self-inflicted injury by the Member (whether the Member is sane or insane); or deliberate exposure to exceptional danger, except in an attempt to save a human life by the Member,
Any Pre-Existing Condition.

Claims Process
Notification of claims:

Death Claim
3 Month’s from date of Member’s death.

Total Temporary or Permanent Disability claim
1 Month from date of disablement.

Critical Condition claim
1 Month from the date the event giving rise to the Member claiming.

Submission of all claim documents
6 months from the date of death. Any additional documentation must be submitted within 60 days of the request by Global Alliance Life.

Claim documents to be submitted for a Death claim:
A completed notification of death claim form,
A completed signed and stamped death claim form,
an original certified copy of the Member’s Identification document or passport,
an original certified copy of the Member’s official death certificate,
an original certified copy of the Member’s last payslip,
an original certified copy of the Beneficiary’s Identification document / passport or birth certificate,
a beneficiary nomination form or an Employer resolution for the distribution of the benefits to the beneficiaries,
an original certified copy of the Member’s latest payslip,
proof of membership
an original certified copy of the beneficiary bank details on a bank letterhead, a copy of the Credit Agreement entered into by the Member,
proof of debt / certificate from the scheme indicating the amount owing at the time of the event giving rise to the claim, and any other documentation as requested by Global Alliance Life.

Claim documents to be submitted for a disability claim
A completed notification of a potential disability claim form,
Section A:
Part 1 - Claimant's personal statement,
Part 2 - Employer’s statement,
Section B:
Confidential medical report,
all available clinical evidence relevant to the claim,
an original certified copy of member’s identity document,
an original certified copy of the member’s payslip for the last completed month of employment,
a copy of the Credit Agreement entered into by the Member,
proof of debt / certificate from the scheme indicating the amount owing at the time of the event giving rise to the claim, and
any other documentation as requested by Global Alliance Life.
Who pays for disability claim evidence?
Initial - Member or employer.
Further - Global Alliance Life.
